Cost for banquet is $75 per person ($140 Couple), payable by check to CAPRCBM or credit card payment through PayPal. Please Note: Mail form and enclose check payable to: CAPRCBM, PMB #157, 1750 Prairie City Road, Suite 130, Folsom, CA 95630.  CAPRCBM does not accept purchase orders. For confirmation, enclose stamped, self-addressed envelope.
